CLIENT REGISTRATION FORM:

Please complete the entire application. During the course of the support provided by ICGA, if any of the information provided changes, you are required to inform us immediately. Failing to do so may result in suspension or termination of all financial support. Financial assistance is distributed on the 1st day of the month. You must submit the application 7 days before this deadline in ordered to be considered (Send by the 23rd day of the month). Applicants must reapply for assistance every month by completing this form. Applicants will be required to submit picture of photo IDs, proof of income (pay stubs), and lease information. Other documents may be requested.
